End-stage renal disease (ESRD) is a major health problem in the United States. Cigarette smoking has been identified as a risk factor for ESRD. However, the consistency of the association between smoking and ESRD has not been established. We conducted a meta-analysis of 16 studies that assessed the association between cigarette smoking and risk of ESRD. Overall, the risk of developing ESRD was increased by 42% among smokers (relative risk [RR] = 1.42, 95% confidence interval [CI] = 1.23 to 1.63). Ex-smokers (RR = 1.65, 95% CI = 1.29 to 2.12) and current smokers (RR = 1.48, 95% CI = 1.26 to 1.74) had an elevated risk of developing ESRD, while former smokers (RR = 1.00, 95% CI = 0.78 to 1.26) did not.
